Applications for the Sacrament of Baptism should be made using the form below.
A separate application form is required for each child.
Baptisms are ordinarily celebrated on Sunday afternoons or Saturday evenings in either Holy Cross or SS Philip and James churches. A request for a baptism on other days and/or at other times will only be considered as an exception.
Please return the completed form to either the parish office or the Presbytery.
